Principle of Toluidine Blue Staining and Chemical Affinity
Toluidine blue is a basic thiazine metachromatic dye with high affinity for acidic tissue components (sulfates, carboxylates, and phosphate radicals); thereby staining tissues rich in DNA and RNA [6]. It has found wide applications both as vital staining in living tissues and as a special stain owing to its metachromatic property [6].
How to Prepare Toluidine Blue Stain Reagents and Solutions
The procedure begins with selecting the correct reagents used, specifically toluidine blue [8]. Dissolve 0.1 g of Toluidine Blue O powder dye (by mixing) in 100 ml of distilled/demineralized water while stirring [6]. Note: The results are pertained to metachromatic staining [6]. Discard the reagent if precipitates appear after staining on smears [9].
Clinical Uses of Toluidine Blue Stain in Pathology
Toluidine blue stains cellular nuclei, and this property makes it useful for identifying malignant tissue, which has an increased DNA content and a higher nuclear-to-cytoplasmic ratio [18]. Toluidine blue is often used to identify mast cells, by virtue of the heparin in their cytoplasmic granules [6]. It is also used to stain proteoglycans and glycosaminoglycans in tissues such as cartilage [6]. Finally, it is utilized for screening of mucosal lesions for dysplasia or malignancy [6].
Toluidine Blue Test Protocol for Mucosal Lesions and Malignancy
In suspicion of mucosal dysplasia or malignancy; e.g. oral mucosal lesions, cervical lesion, etc. this test is carried out as follow [18]. Wipe mucosal area suspicious for carcinoma with 4 x 4 soaked in acetic acid solution [18]. Apply toluidine blue solution [18]. Blot dry. The mucosal part which is not taking blue colour is the suspected area and biopsy should be obtained from that area [18]. The sensitivity and specificity of toluidine blue test for the detection of malignancy was 92.6 and 67.9% respectively; and the overall diagnostic accuracy was 80% [18]. The result was highly significant with a ‘p value’ <0.001 [18].

Let the smears should dry before observing under microscope under low lower and high power [13]. Monnier technique: Spray the target area with 1% N-acetylcysteine solution [13]. Wait for 1 minute [13]. Then spray the area with toluidine blue solution [13]. Wait 2 minutes [13].

Stain sections in toluidine blue working solution for 2-3 minutes [1]. Wash in distilled water – 2 mins [1]. Dehydrate quickly in 95% ethanol [1]. Dehydrate in 100% alcohol (10 dips each) [1].
