Overview

TB Gold Quantiferon is an interferon-gamma release assay used as an aid in the diagnosis of latent tuberculosis infection and active tuberculosis. The test measures the immune response of sensitized T-cells when exposed to Mycobacterium tuberculosis-specific antigens. It uses whole blood to detect interferon-gamma released after stimulation with ESAT-6, CFP-10, and TB7.7 antigens, which are absent in BCG vaccine strains. This makes the test highly specific and useful as a replacement or supplement to the tuberculin skin test. Early detection using this assay supports timely diagnosis and helps in controlling the spread of tuberculosis.

Symptoms

The TB Gold Quantiferon test does not detect symptoms directly but supports evaluation in individuals suspected of tuberculosis infection. Patients tested may present with symptoms suggestive of active tuberculosis, such as prolonged cough, fever, weight loss, or fatigue, or may be asymptomatic in cases of latent infection. The test helps identify immune sensitization to tuberculosis antigens even when clinical symptoms are minimal or absent, especially in latent TB cases.

Causes

A positive TB Gold Quantiferon result occurs due to infection with Mycobacterium tuberculosis. When an individual is infected, T-cells become sensitized and release interferon-gamma upon exposure to TB-specific antigens during testing. Unlike the tuberculin skin test, prior BCG vaccination does not cause false-positive results. The test response reflects immune recognition of tuberculosis antigens rather than direct detection of the bacteria itself.

Risk Factors

Individuals at increased risk of tuberculosis infection include those with close contact with active TB cases, healthcare workers, and people living in high TB prevalence settings. Immunocompromised individuals, including those receiving anti-TNF or other immunosuppressive therapies, are also at higher risk. Patients being evaluated for sarcoidosis may undergo TB Gold Quantiferon testing due to the proposed association of Mycobacterium tuberculosis with the condition. BCG-vaccinated individuals benefit from this test due to its higher specificity.

Prevention

Early identification of latent tuberculosis infection using TB Gold Quantiferon helps prevent progression to active disease through timely treatment. Screening high-risk populations, healthcare workers, and individuals prior to immunosuppressive therapy plays a key role in prevention. Contact tracing after TB exposure and routine screening in BCG-vaccinated individuals reduce missed diagnoses. Prompt diagnosis and monitoring using this assay support effective TB control strategies and help limit disease transmission.
